CARE PLAN

Create a Custom Care Plan

Based on the assessment, we’ll design a personalized care plan that includes services like personal care, companionship, transportation, meal prep, and more. Every plan is flexible and built around your family’s preferences, schedule, and budget.

CAREGIVER

Match with the Right Caregiver

We carefully match your loved one with a professional caregiver whose personality, skills, and experience align with your specific care needs. All of our caregivers are vetted, trained, and supported by our leadership to ensure quality and consistency.

CONNECT

Begin Care & Stay Connected

Once care begins, we remain in close communication with you to ensure everything is going smoothly. Our team conducts regular check-ins, care updates, and plan adjustments as needed—so your loved one always feels supported, and you always feel informed.

At Aging with Care, our caregivers are more than employees—they are compassionate professionals devoted to enhancing the lives of those they serve. Hand-selected for their experience, empathy, and reliability, each caregiver represents our core belief: to make a difference, you have to care.

Our Caregivers

  • All caregivers at Aging with Care undergo comprehensive training to ensure they are equipped to support clients with safety, dignity, and professionalism. Training includes personal care techniques, communication skills, mobility assistance, and emergency response. All caregivers also complete a thorough background check prior to joining our team.

  • Where required by state regulations, our caregivers hold valid home care aide licenses or certifications, ensuring compliance with California’s in-home care standards.

  • Many of our team members carry additional certifications in areas such as CPR, dementia care, and hospice support—bringing specialized knowledge to sensitive care situations.

  • We conduct ongoing evaluations, client feedback reviews, and supervisory check-ins to ensure the highest level of service and consistency. Care plans are routinely reviewed and adjusted as needed to match evolving client needs.
